We are pleased to accept most major PPOs, some HMOS, Medicare and IEHP.

For a current list of approved insurance providers and to confirm yours is accepted, please call our office at 1.800.VARICOSE (1-800-827-4267). Procedures that are medically necessary with a diagnosis are normally covered.

Treatments for varicose veins, including VenaSeal™ procedure, ClosureFast™, ambulatory phlebectomy, and sclerotherapy are covered by almost all plans.

Procedures for cosmetic purposes only are not typically covered by insurance.

your first visit

If you’re visiting our vein center in Riverside for the first time, please arrive 15 minutes before your scheduled appointment time to complete all necessary forms. Or download them here and bring the completed forms to your appointment.

What to Bring to Your First Appointment

We encourage all patients to bring the following to their first office visit:

  • Insurance card(s) and related information
  • Referral documents
  • List of current medications
  • Co-payment, if applicable
  • Completed Pre-registration Patient Forms

Please drink at least 6 to 8 glasses of water before your consultation since we will perform an ultrasound as part of your visit.

Please call our office if there are any changes to your name, address, telephone numbers, or insurance so that we can keep your records up to date.
